MAP works closely with individuals to secure Medium Term Accommodation that enables them to live safely and securely whilst waiting for the transition to long-term housing solutions.
Options are explored, and the best match for housing and support is developed around your needs and wants.
Specialist Disability Accommodation (SDA) is a range of housing designed for people with disability that have extreme functional impairment or very high support needs. The SDA dwellings have accessible features to help residents live more independently and allow other supports to be delivered safely.
My Ability Pathway in partnership with New Dawn Housing will have capacity to offer support and SDA housing in 2024.
MAP Support Coordination services are delivered by a diverse team of individuals with a range of expertise, knowledge and lived experience. They will work with you to identify the right mix of supports and make the best use of your funded supports in your plan. Support Coordination is a capacity building support that will assist you to understand your NDIS plan and engage the right NDIS providers and services based on your circumstances, needs and preferences.
Your Support Coordinator can also utilise your funding to assist you in finding suitable independent home and living supports.
MAP offers Specialist Support Coordination to people with more complex needs. The focus is on reducing complexity in the support environment and helping to overcome immediate and significant barriers. Our specialist support coordinators identify and understand the obstacles impacting an individual’s ability to access, use and maintain their support, leading them to find and try solutions to address these barriers.
A recovery coach provides people with a psychosocial disability a more targeted service and assists with developing their personal recovery plan. Our Recovery Coaches have lived experience and a better understanding of mental health challenges within a recovery-oriented practice. They are there to help you plan for the good days and the bad days.
MAP offers short term accommodation and respite in the stunning Barossa Valley. Our short-term accommodation and respite service provides the comforts of quality hotel accommodation in a private home, supported by experienced, caring staff. The MAP team will work with you to plan your stay including, how long you will stay, what support you may need during your stay and what activities you would like to be involved in. You can choose to relax and unwind or immerse yourself in the endless adventures available.
MAP can also provide alternative options for short term accommodation and respite. A change of environment might be all you need for you and your informal supports to relax and recharge.
