Diagnosed in 2019 with young onset dementia, Michael quickly found himself unable to remain safely in his own home. As his disability progressed, Michael faced the risk of becoming homeless. Initially, Micheal moved into a residential aged-care facility. This was not a suitable long-term option, so Michael was supported by his family and a Support Coordinator from My Ability Pathway (MAP). Each played a crucial role in supporting Michael through the process of exploring Supported Independent Living (SIL) and SDA funding through the NDIS. Today, he lives in a private home while waiting for his SDA dwelling to be built in his community.
In September 2023, 64-year-old Simon* experienced a stroke that left him unable to return home safely. While being moved between multiple hospitals for treatment, Simon applied to become a participant of the NDIS. His Support Coordinator suggested asking for Specialist Disability Accommodation (SDA) funding as part of his first plan. Simon’s SDA application was successful, and he has now moved into a new, accessible home that lets him live with dignity and independence.
